			<content:encoded><![CDATA[<p>This was the last day of the Physician Board Exam and it will be 3 days to go before the release of the <strong>Physician Licensure Examination February 2011 results by the PRC</strong>, and a deep sigh of relief is expected of every examinees.  However, the waiting period will be more stressful than the examination proper itself, and believe me it will be the longest 3 days of your life. You will tend to eat less, sleep less and talk less.</p>
<p>Subscribe here to be the first to know as soon as the <strong>PRC Physician Licensure Examination Results Feb  2011</strong> are released.  The topnotchers for the <a title="February 2011 Physician Licensure Examination Results" href="http://iamjammed.com/education/physician-licensure-examination-february-2011-results/"><strong>February 2011 Physician Board Exam</strong></a> will be posted here as soon as the results are released by the PRC.</p>

			<content:encoded><![CDATA[<p>Acquiring an S2 License Number enables a physician to prescribe regulated drugs such as anti psychotics and antidepressants.  Applying for an S2 license number is easy if you have already prepared your requirements which include the following:</p>
<p>1. PRC ID<br />
2. Latest PTR<br />
3. TIN Card or ITR<br />
4. Latest Drug Test Result, which should be <strong>negative</strong> and computerized<br />
5. Two (2) pcs 1&#8243; x 1&#8243; ID picture with name tag, no eyeglasses and taken not later than a month from application of any background color.<br />
6. PhP1500 fee, valid for 3 years.</p>
<p>For renewal, the requirements are basically the same except that you must present your S2 ID card. For lost S2 ID card, submit Proof of Publication of Loss.</p>
<p>They require a photocopy of each of these documents or requirements however you have to bring the originals for verification. </p>

<p>The Dengue cases in the Philippines are continually rising especially around the Metro Manila area.  The latest count of death toll from dengue reached 501 as of yesterday.  With this increasing number of mortality,  the Department of Health had to formulate a strategy to decrease this number.</p>
<p>The Department of Health has recently devised their new D.E.N.G.U.E.  strategy. Instead of confining patients in a hospital facility, parents  and  caregivers can practice the D.E.N.G.U.E. strategy. D.E.N.G.U.E.  stands  for:<br />
D &#8211; daily monitoring of patient’s status,<br />
E &#8211; encourage intake of  oral fluids like oresol, water, juices, etc,<br />
N – note any dengue warning  signs like persistent vomiting and bleeding,<br />
G – give paracetamol for  fever and NOT aspirin, because aspirin induces bleeding,<br />
U – use  mosquito nets and<br />
E – early consultation is advised for any warning  signs,</p>
<p>This new strategy aims to educate the  public on home treatment of mild  dengue cases which comprise a greater part of the number of patients  with dengue. This is to also help  decongest hospitals by giving an  assurance that not all dengue cases  require hospital confinement but  can be managed at home using the said strategy.</p>

			<content:encoded><![CDATA[<p>Time flies so fast.  Four years ago, I was a struggling medical student.  I couldn&#8217;t believe that I actually managed to surpass the trials medical education entailed.  I will be an intern in the next 2 weeks! But I don&#8217;t know if I should breath for relief or sigh for anticipation and worries.  I know that finishing my four years in medical college and entering internship mean that I am facing a tougher life and a greater responsibility.  </p>
<p>When you are an intern, you are expected to have acquired most, if not all, the knowledge you need from your four years in medical education &#8211; that&#8217;s a big responsibility you say. Being admitted to a national hospital (in Jose Reyes Memorial Medical Center) makes me worried all the more. </p>
<p>However, Nietzche&#8217;s said that &#8220;He who has a why to live for can bear almost any how.&#8221; This has been my motto in life ever since I started my medical education.</p>
<p>I am crossing my fingers for this new chapter of my life.  I know prayers and perseverance will do the trick.</p>

			<content:encoded><![CDATA[<p>I remember during one of the meme tags I have received about the <a href="http://blog.jammedph.com/tagged-and-8-random-facts-about-me/">8 random things about me</a>, I said my left eye is not functioning anymore. And it was sad but true. It&#8217;s called &#8220;lazy eye&#8221; or in medical term <a href="http://en.wikipedia.org/wiki/Amblyopia">amblyopia</a>. It is a childhood disease. I have actually detected this problem since I was about 7 years old during our yearly eye exam in elementary grade. I told my mom about it but she did not want to entertain it because she was saying I couldn&#8217;t have a &#8220;blind eye&#8221; at a young age. Well, it&#8217;s one of the misconceptions moms have sometimes. The case has gotten worse over years and now, I only see blurry images with my left eye. Good thing I have my right eye, but I have to wear eyeglasses and contact lenses.</p>
<p>During our clerkship rotation in the EENT department, I was examined by our consultant and officially said I have amblyopi. He has saying that I can have a <a href="http://en.wikipedia.org/wiki/LASIK">laser vision correction (LASIK)</a>.</p>
<p>The procedure is costly and there are complications I should consider. One scary complication is irreversible blindness.</p>
<p>Well, to answer my own question. I badly need my vision and LASIK may help me to gain it back. </p>

			<content:encoded><![CDATA[<p>During my hospital and clinic exposures as a senior medical clerk, I&#8217;ve encountered a lot of patients who switched physicians because of one reason: drug side effects but their doctors failed to mention it to them.  For an average layman, he would think that he has been inadequately informed, which was true at some point and the result is broken trust to the physician, especially if he has learned that side effect from a neighbor who claims to have a &#8220;similar&#8221; experience.</p>
<p>Recently, my fathers&#8217; blood pressure has been fluctuating. His blood pressure was always ranging from 130-140/80-100 mmHg and was maintaining an anti-hypertensive drug. However, recently, his blood pressure rises up to 160/100 mmHg, so he decided to consult his physician who added another anti-hypertensive to his maintenance drugs. </p>
<p>But my father has been complaining to me about his cough which was seemingly aggravated by his additional drug. I explained to him that the drug&#8217;s most common side effects is coughing. I&#8217;ve been convincing him to go to his physician but refuses to and decided to switch to another internist due to that reason.<br />

<strong>Why is it always important to explain everything to your patient?</strong><br />
- It is one way of building a patient-doctor relationship, which is at the very fulcrum of care.</p>
<p>- In the advent of the internet, every information (including misleading ones) is available to the patient. He will believe everything he reads.  Yesterday, we had a patient who could explain the mechanism of action, side effects, dosage, etc. of the drug she was taking. </p>
<p>- You gain trust and respect. Your patients&#8217; perceived that they mattered to you (as a doctor), whatever their background, way of life or philosophy. You will then be recommended to other patients. </p>

			<content:encoded><![CDATA[<p>One of our juniors asked which is better for training or internship, private or public hospital? And this question has been in my head also since the start of our post-graduate internship application.</p>
<p>I have been exposed to both private and public hospitals during our clerkship. I am now at the ancillary department so I am already out of hospital duties (yehey!).</p>
<p>To answer the question, I had to ask these things to myself:</p>
<p>1. Do I prefer toxic or benign duties?<br />
Public hospitals cater for the common people so it is expected that you&#8217;ll have toxic duties. But in return, you are exposed to more learning: you get to encounter several cases, including the rare ones. On the other hand, private hospitals are expectedly benign, well except maybe for some selected large hospitals.<br />

During our clerkship, I&#8217;ve experienced managing patients on my own, even the complicated ones like cerebrovascular accidents, myocardial infarctions, etc. at the public hospital where I trained to. It was a great feeling to know that what you have been doing to your patient was the right management. We do the admitting orders, admitting notes, complete history and PE, discharge summaries, etc. Unlike in different school, our paperworks are attached to the chart, not only for the sake of completion of requirements but as bases for management of the patient. While at a private hospital, we were only allowed to take an &#8220;unofficial&#8221; history and PE (the ROD makes the official one), insert it to the patient&#8217;s chart and then you&#8217;ll see several months after that it was used to wrap a bunch of cotton pledgets. Heartbreaking, ain&#8217;t it? Especially if you have received extensions because the 24-hour period has lapsed.</p>
<p>2. Do I prefer to know the ideal management?<br />
But of course, who wouldn&#8217;t? They say that you get the ideal management at a private hospital because you will have the complete workup for a patient&#8217;s case, therefore confirming your diagnosis and ruling out your differentials. Although, I am not talking in general. This case is also possible in big public hospitals, especially in training ones. I&#8217;ve heard some feedbacks though that oftentimes, you have to pay for the laboratories if the patient can&#8217;t afford.</p>
<p>3. Do I prefer getting skills or enhancing my knowledge?<br />
I answered both. At this stage, I admit I still lack some clinical and basic knowledge. So I want to learn more, but I want to accumulate skills also in preparation for future residency training. </p>
<p>4. Should I consider financial advantage?<br />
I consider this as the least factor to affect my choice. My focus now is learning. Some private hospitals offer allowances and privileges such as free dorm and meal while rarely do you find a public hospital (if there is) which offers similar advantage.  </p>
<p>So after answering those things, I have come up with my three choices, all public hospitals:</p>
<p><a href="http://www2.doh.gov.ph/jrrmmc/jrhome.htm">Jose Reyes Memorial Medical Center</a>.  I should expect toxic duties they say but you learn from your everyday exposure with different cases. It has no financial advantage or any privileges, though. Similarly, I feel the same thing for Davao Medical Center and Northern Mindanao Medical Center in Cagayan de Oro City.</p>
